This page is where you can find all the information you need for your first appointment. We want to provide you with a brief orientation on what to expect on your first appointment, as listed below. Please be sure to bring the new patient paperwork, some identification, and your insurance card.

Please download and complete the New Patient Paperwork and bring it on your initial visit.

What to expect

Upon Arrival

You will be greeted by your physical therapist and given a tour of the facility so you can feel at home. We will then discuss your insurance benefits, so there are no surprise expenses, and get started with the examination.

Examination

We will perform a comprehensive initial examination to determine the source of your pain and problems. We then will then create an individualized treatment plan to help you reach your goals and reduce the dysfunction that is causing your pain.

What to wear?

Wear something comfortable that will allow you to move freely so we can address your areas of pain. We recommend wearing supportive shoes that you can exercise in.

What to bring?

Please bring or email a copy of the New Patient Paperwork, your insurance card, and a form of identification.
